Comments:

An unannounced renewal inspection was conducted today from 12:00pm-3:40pm. 37 children were observed directly supervised by 8 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, and policies were inspected. Children were observed during naptime and playing quietly. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Standard #:	22.1-289.035-A
Description:	Based on record review, the center did not ensure 2 of 4 staff undergo background checks every five years. Evidence: 1. The most recent fingerprint-based background check in the record for Staff #1 was dated 10/16/2023, more than five years after the previous fingerprint check dated 7/12/2018. 2. The most recent Central Registry Search results were dated 10/06/2023, more than five years after the previous Central Registry Search results dated 3/12/2018. 3. The most recent sworn statement in the record for Staff #1 was dated 1/11/2018. 4. The most recent fingerprint-based background check in the record for Staff #2 was dated 10/16/2023, more than five years after the previous fingerprint check results dated 5/14/2018. 5. The most recent Central Registry Search results were dated 10/06/2023, more than five years after the previous Central Registry Search results dated 5/31/2018. 6. The most recent sworn statement in the record for Staff #2 was dated 4/09/2018.
Plan of Correction:	Immediately upon learning the background checks had expired, new background checks were submitted. The updated background checks are located in the records.

Description:	Repeat Violation Based on record review the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the first date of attendance for 2 children. Evidence: 1. Child #4 began attending the center on 1/30/2023. The record for Child #4 did not contain documentation of immunizations received by Child #4. 2. Child #2 began attending the center on 1/3/2023.The center could not verify the child has received immunizations required by the State Board of Health. The document provided was not in English.

Description:	Repeat violation Based on record review, 2 of 4 children's records reviewed did not have documentation of a physical exam with 30 days of the child's first day. Evidence: 1. Child #4 began attending the center on 1/30/2023. The record for Child #4 did not contain documentation of a physical exam. 2. Child #3 began attending the center on 1/3/2023. The medical documentation in the record for Child #3 was not in English. The center could not verify completion of a physical exam as required based on documentation received.

Description:	Based on record review, 3 of 4 children's records did not contain all required information. Evidence: 1. The record for Child #1 did not contain the name and address for one of two designated people to call in an emergency if a parent cannot be reached. 2. The record for Child #3 did not contain the work phone number and place of employment of one parent, and the work phone number of the second parent. 3. The record for Child #4 did not contain the work phone number and place of employment of one parent, and the work phone number of the second parent. 4. The record for Child #4 did not contain documentation of viewing proof of the child's identity and age.

Description:	Based on observation, the center did not ensure there was at least 12 inches of space between occupied cots. Evidence: 1. Multiple occupied cots were observed closer than 12 inches apart in the 3year old and 4 year old classrooms.
Plan of Correction:	Furniture was rearranged to make sure to space the cots out a little more.

Description:	Based on observation, 1 of 6 cots observed did not consist of a top cover. Evidence: 1. One child was observed sleeping on a cot without a top cover.

Description:	Based on review of medications, medication was given to a child without written authorization from the parent. Evidence: 1. The medication authorization in the record for Child #6 expired on 8/4/2023. The medication was administered to the child on 9/18/2023, 9/20/2023, 9/25/2023, 9/27/2023, 10/02/2023, 10/04/2023, 10/11/2023, 10/16/2023, 10/23/2023.
Plan of Correction:	Paperwork was given to the parent to have completed and parents were given the medication until they provide authorization.
Standard #:	8VAC20-780-510-P
Description:	Repeat Violation Based on a review of medications, the center did not follow procedures regarding expired medication authorizations. Evidence: 1. The medication authorizations for two medications prescribed to Child #5 expired on 7/17/2023 and 7/23/2022. Medications were not picked up by the parent within 14 days or destroyed by the center. 2. Three medications prescribed to Child #2 expired on 8/2023, 2/2023, and 12/2021. The medications were not picked up by the parent or destroyed by the center.
Plan of Correction:	The center has requested updated authorizations, and anything expired has been sent home.
