Technical Assistance:
Discussed in detail the requirements related to Playground Equipment-Resilient Surfacing (8VAC20-780-330) regarding spring rockers and merry- go-cycles.

Comments:
An unannounced monitoring inspection was started in the center on November 15, 2023 from 2:15 PM until 4:05 PM and concluded on November 16, 2023. At the time of entrance, 16 children were in care with 4 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed eating snack, sleeping , playing freely, during diaper changes and reading books.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-150-B
Description: Based on record review and provider interview, the licensee did not ensure the physical examination and dates of immunizations shall include the signature of a physician, his designee, or an official of a local health department

Evidence: The immunization and physical documents for Child #5 did not include a physician or designated individual?s signature.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review and staff interviews, the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The attendance sheet for Infant Room was not accurate, it did not include Child #1 who was observed in care.

Description: Repeat Violation
Based on observation, the licensee did not ensure hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the infant room there were containers of hazardous substances, a spray disinfectant and bottle of hand sanitizer were stored on a shelf in an unlocked manner. These items were labeled "keep out of reach of children" and at least one other statement "caution", "flammable" and "warning".

Description: Based on observation, staff interview and record review, the licensee did not ensure to
follow the required staff-to-child ratio for children birth to 16 months is 1:4.

Evidence: On 11-15-2023, in the infant room, from approximately 2:15 PM until 2:25 PM, there were 9 infants with 2 staff.

Description: Repeat Violation
Based on observation and staff interviews, the licensee did not ensure children's and staff hands shall be washed with soap and running water or disposable wipes when required.

Evidence: The following were observed
1) Child #4 and Child #3 did not have their hands washed with soap and running water or disposable wipes before and after eating meals or snacks.
2)Staff #4 was conducting a diaper change for Child #7 and Child #8 and they did not wash their hands in between diaper changes and did not ensure to wash Child #7 and Child #8?s hands after their diaper change. Staff are required to wash their hands with soap and running water after toileting/diapering and ensure that children?s hands are washed with soap and running water after toileting/diapering.

Standard #: 8VAC20-780-500-B
Description: Repeat Violation
Based on observation and staff interviews, the licensee did not ensure the required procedures for diapering were followed.

Evidence: The following were observed in the toddler room.
1) The diapering surface is required to be cleaned with soap and at least room temperature water and sanitized after each and this was not followed. Staff #4 was conducting diaper changes for Child #7 and Child #8 and after each diaper change they wiped down the diaper pad with a disinfect wipe and then sprayed it with a disinfectant and immediately changed another child?s diaper. Sanitized is defined as sprayed with a disinfectant /sanitizing solution and allowed to air dry or wiped down after two minutes.
2) The requirement that disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal was not followed. Staff #4 was changing diapers for Child #7 and Child #8 and immediately after each change they placed the soiled diaper in plastic bag that that they had to open with their hands.

Description: Based on observation and staff interviews, the licensee did not ensure when a child is placed in a highchair, the protective belt shall be fastened securely.

Evidence: Child #2 was observed sitting in a feeding table and the protective strap was not used
