Comments:
An unannounced monitoring inspection was conducted on 11/15/23 from 11:47 a.m. to 1:14 p.m. with the Assistant Director. There were 46 children in care, ranging in age from 12 weeks to five-years-old, supervised by nine staff. The children were observed napping. Five child records and five staff records were reviewed. Seven medications and authorization forms were reviewed and the center has seven staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. The most recent Fire Inspection on file was dated 12/31/23 and the most recent Health Inspection on file was dated 11/04/22. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that not all areas of the center inside and outside were maintained in a clean, safe, and operable condition

Evidence: In the two-year classroom, the bathroom had screws protruding upward from the base of the toilet that extended approximately 12 thread lengths.

Plan of Correction: The original toilet screws will be cut as requested.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurements, it was determined that the slide on the playground did not have the required amount of resilient surfacing underneath and surrounding the structure.

Evidence:
1. The slide requires 6 inches of shredded rubber in front and on all sides of the slide, the shredded rubber was approximately 1-1/2? at the slide exit.
2. There was approximately 3 inches of shredded rubber in the fall zone of the four swings. Six inches of shredded rubber is required in the front and back fall zone of the swings.

Plan of Correction: The rubber mulch will be raked to evenly distribute it to the most used areas. As part of regularly scheduled maintenance, more mulch will be added if needed.
