Discussed with staff member in charge: moving children up to a new room, discussion of the letter from DOE regarding the practice of moving children into the Children?s House rooms prior to the age of 3, completion of injury report forms, food from home labeled and dated.

An unannounced, focused monitoring inspection was conducted today to follow up on previous violations. The Head of School was not present today.
Children were observed playing in their rooms and involved in a variety of activities.
1 staff record requested for review was not available during the inspection. Children?s complete records were not available (Proof of Birth documentation).
Please send the staff file requested, (specific items only) by the Close of Business (COB) tomorrow. (5:00PM)
Based on the number of repeated violations and systemic deficiencies, Administrative staff will be contacted to discuss an Intensive Plan of Correction (IPOC).
