Technical Assistance given and licensing standards reviewed regarding children?s records and emergency numbers.

An unannounced monitoring inspection was conducted on November 20, 2023. The inspector arrived at 10am and departed at approximately 11:15 AM. There were 4 children present and 4 staff members. A sample size of 10 records were reviewed which included 5 children?s records and 5 staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, free play, and outdoor play. There are no medications at the center and transportation is not provided at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Standard #: 8VAC20-780-160-A Description: Based on record review and interview, it was determined that the center did not ensure that each staff shall submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. Staff #2 (date of hire October 2, 2023) has a negative tuberculosis screening in their record dated for March 4, 2022, which is outside of the required 30 days of the date of employment.
2. Staff #1 confirmed that Staff #2 has a negative tuberculosis screening outside of the last 30 calendar days of the date of employment.Plan of Correction: The center responded with the following: Staff #2 will complete a new tuberculosis screening and place the results in their record at the center.
