Comments:
An unannounced monitoring inspection was conducted on November 21, 2023, from 10am to approximately 12:30pm. There were 14 infant children present and 4 staff members. The Inspector reviewed five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during morning snack, bottle feeding, diaper changes, free play, and group activity. There is no transportation provided for the infants at this time at the center. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation was discussed with the owner at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center shall be maintained in a safe and operable condition.
Evidence:
1. The large fish tank stand located in the infant classroom has two cabinet doors with no protective safety lock that are in reach of children. Children at the center can reach and open it which poses as an injury hazard.
2. Infant staff members confirmed that there was no safety lock on the two cabinet doors for the fish tank stand and that children can reach and open the cabinet doors.
3. The electrical cords from the swings to the electrical outlet were exposed and loose for children to grab when crawling on the ground. This poses as a choking, tripping, and injury hazard to the children in case.
4. The infant staff members confirmed that the electrical cords to the swings were loose and were accessible to the children in care.

Plan of Correction: The center responded with the following: The center will place a safety lock on the two cabinet doors of the fish tank stand to ensure that children can no longer open and close the door. The center will secure the cords to the swings to ensure that the children cannot grab, trip, or injury themselves. They will ensure that there is no access for the cords.
