Technical Assistance:
Technical Assistance given and licensing standards regarding spacing and outdoor play were reviewed.

Comments:
An unannounced monitoring inspection was conducted on November 21, 2023, from 10am to 12:30pm. There were 21 children and 6 staff members present. The Inspector reviewed five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, lunch, free play, and naptime. There is no transportation provided at this time. There is construction going on at the center however the entire right side of the building is blocked off with no access. Walls have been taken down to have a larger toddler room for the future. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the owner at the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, it was determined that the center did not ensure that each staff member submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #2 (date of hire 2/22/2023) did not have a negative tuberculosis screening document in their file.
2. Staff #1 stated that Staff #2 was a re-hire, and the tuberculosis screening document was not in their file.

Plan of Correction: The center responded with the following: Staff #1 will have Staff #2 complete a new tuberculosis screening and place the results of a negative test in their record.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, it was determined that the center did not ensure that the diapering area shall have a nonabsorbent surface for diapering or changing shall be used.
Evidence:
1. The changing pad used for children in care has a large tear to the waterproof lining. Due to the tear, this is not a nonabsorbent surface for diapering.
2. The staff members in the classroom confirmed the tear to the waterproof lining.

Plan of Correction: The center responded with the following: The changing pad will be disposed of, and a new changing pad will replace the old one. Staff will ensure that it is always a nonabsorbent surface for diapering.

Standard #: 8VAC20-780-550-G
Description: Based on record review and interview, it was determined that the center did not ensure that documentation be maintained of emergency evacuation, shelter-in place, and lockdown drills that includes: 1. Identity of the person conducting the drill; 2. The date and time of the drill; 3. The method used for notification of the drill; 4. The number of staff participating; 5. The number of children participating; 6. Any special conditions simulated; 7. The time it took to complete the drill; 8. Problems encountered, if any; and 9. For emergency evacuation drills only, weather conditions.
Evidence:
1. In reviewing the emergency evacuation drill logs, the date and time of the drill were the only components listed to the log.
2. Staff #1 confirmed that on their emergency evacuation drill logs, it was missing the identity of the person conducting the drill, the method used to conduct the drill, the number of participants, the number of children participating, any special conditions, the time to complete the drill, and the weather conditions.

Plan of Correction: The center responded with the following: Staff #1, moving forward will document all the required components for the drills and use the model form from the DOE website.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, it was determined that the center did not ensure that when food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner.
Evidence:
1. All the lunch boxes in the refrigerator did not have a clearly labeled date.
2. Staff #1 confirmed that there were no dates on the children?s lunchboxes.

Plan of Correction: The center responded with the following: The center will inform the parents that dates need to be on the lunchbox. The center will also have labels provided to date
