Comments:
An unannounced renewal inspection was conducted onsite on 11/20/2023. Records and other required documents were reviewed at the administrative office on 11/20/2023. There were 5 children, ages 7 years - 12 years, and 3 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during self-directed activities, snack, and departure. A total of 5 children?s records, and 4 staff records were reviewed. The inspector discussed the following with the provider: playground policy and fire inspection. The inspector arrived for the inspection at approximately 2:50 pm and departed at approximately 3:50 pm.
The information gathered during the inspection determined non- compliance with applicable standards or law and violations were documented on the violation notice to the provider.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview with staff, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.

Evidence: The record for Child 5 did not contain a first day of attendance. Staff 2 confirmed the first day of attendance in February 2023, exact date unknown. There was no physical examination for review.

Plan of Correction: Provider will obtain from parent.

Standard #: 8VAC20-780-60-A
Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.

Evidence:
1. The record for Child 1 did not contain the following information: name and phone number of physician, name address and phone number of two designated people to call in an emergency if a parent cannot be reached (no addresses for both designated people), names of persons authorized to pick up the child, and chronic physical problems and pertinent developmental information and any special accommodation needed.
2. The record for Child 3 did not contain the following information: written parent agreements between the parent and the center.
3. The record for Child 5 did not contain the following information: sex of the child, name and phone number of physician, name address and phone number of two designated people to call in an emergency if a parent cannot be reached (no addresses for both designated people) and first day of attendance.

Plan of Correction: Provider will update child forms.

Standard #: 8VAC20-780-260-A
Description: Based on record review, the center failed to provide the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence: The most recent fire inspection available during the inspection was dated 05/24/2022. There was not a fire inspection for 2023 available for review.

Plan of Correction: Provider will contact local fire officials.

Standard #: 8VAC20-780-550-G
Description: Based on record review, the center failed to ensure that documentation of emergency evacuations, shelter-in place and lockdown drills shall include all the information required by the standards.

Evidence: The documentation for emergency evacuation did not include the following information: identity of person conducting the drill, number of staff participating and weather conditions.

Plan of Correction: Provider will update form.
