Comments:
An unannounced renewal inspection was conducted today from 12:10pm-1:50pm. There were 36 children (2-5yrs.) directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, story time, singing, eating and napping. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review, the center did not obtain fingerprint results for a staff member prior to their first day of employment.

Description: Based on record review, the center did not obtain acceptable documentation of a negative tuberculosis screening for a staff member at the time of employment and prior to coming into contact with children.

Evidence:
Staff #5's record did not contain a clearance statement signed by a physician, the physician's designee or an official of the local health department, stating that the individual does not have any current symptoms of active tuberculosis, a risk factor for acquiring tuberculosis infection or a risk factor for progression to active tuberculosis disease as defined by the local health department.
