Technical Assistance:
1. Consultation provided on ensuring that all staff hired since 10/21 have obtained those new components of orientation training, please refer to standard 240.C 1-11

2. Make sure staff are consistently documenting time of day parents were notified of injuries/accidents.

3. Consultation provided on locations of fans and the need to ensure the classroom space is safe for children.
4. Continue to remind staff that the phone cords in the classroom must be out of reach of the children.
5. Please make sure the staff in the Prek B classroom are immediately addressing and intervening when children are roughhousing.
6. Staff should be assessed each outside play space prior to the children using the equipment to ensure nothing is stored in the fall zones of the stationary equipment, this includes balls, tricycles or an object.

Comments:
An unannounced monitoring inspection was conducted today with the director from 9:40 am to 12:40 pm. Seventy-six children were present with 13 staff providing supervision. The ages of the children ranged from 2 months to 5 years. Children were observed outside on the playground, circle time in the toddler classroom where the children were singing baby shark, infants playing on the floor and napping in their cribs.Two vans were inspected during this inspection as well as medications for 7 children, 6 staff and children's files and 4 allergy care plans. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of six children's files and interview with administrative staff, it was determined that there was no documentation of immunization for each child before the child can attend the center.
Evidence:
Child A did not have an immunization record on file. Child A was enrolled on 10/9/23.

Plan of Correction: We will get the immunization record.

Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that the area and equipment was not maintained in an safe condition.
Evidence;
The Licensing Inspector observed a fan high on an upper shelf in the Preschool IA room, The cord to the fan was hanging in reach of the children in that classroom therefore presenting a potential hazard of a child pulling on the cord and the fan falling on top of a child(ren).

Plan of Correction: We will immediately remove the fan from the shelf and retrain staff on making sure their classrooms are maintained in a safe manner.

Description: Based on observation, it was determined that hazardous substances were not kept locked.
Evidence:
The Licensing Inspector observed spray bottles of sanitizing agents in the kitchen on a cart. The door to the kitchen was open. The Licensing Inspector observed a disinfectant spay unlocked and on a sink in the City Place bathroom.

Plan of Correction: We will lock the hazardous substances.

Standard #: 8VAC20-780-440-J
Description: Based on observation, it was determined that there was not 30 inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Evidence:
There were 3 cribs, two of which were occupied and all 3 cribs were touching each other on the service sides of the cribs.

Plan of Correction: We will space the cribs to at least 30 inches on the service sides.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medications for 7 children, it was determined that there was no parental authorizations for medications for 2 children.
Evidence:
There was no parental authorization for an inhaler for child B or an allergy medication for child C

Plan of Correction: We will get parental authorizations.
