Comments:
An unannounced renewal inspection was conducted on 11/15/2023 from 10:50 am to 2:30 pm. There were 45 children in care, ages 16 months to five years, in four groups under the care of eight staff. The Licensing Inspector reviewed five children?s records, seven medications and associated records, five staff records, and other program records. The children were observed on the playground, indoor group activities, and getting ready for lunch.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the center failed to maintain staff to child ratio in a group of children.

EVIDENCE:
12 children aged three and four were observed on the playground with one staff present. The required ratio for this age group is one staff for every 10 children.

Plan of Correction: From now on, an additional staff person will be available to maintain ratios when a staff person leaves the group to do other tasks.

Description: Based on record review, the center failed to have current permission to administer medication to children.

EVIDENCE:
1. Permission to administer medications 1 and 2 to Child 3 expired October 2023.

2. Permission to administer medications 3 and 4 to Child 4 expired October 2023.

3. Permission to administer medications 5 and 6 to Child 5 expired 7/19/2023.

Plan of Correction: The center will have parents sign a 10-day permission slip while they wait for the physician?s permission to administer for a longer length of time.
