A monitoring inspection was conducted on 11/20/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: (Repeat Violation) Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. A baby was observed sleeping in a crib that had loose sheets, posing a suffocation hazard. There were 3 other cribs that had loose sheets.
2. There were protruding screws in the bathroom stalls and a hole in the wall where the door knob hits the wall, exposing drywall and insulation.Plan of Correction: The center will ask parents to bring in portable cribs sheets and staff will ensure the sheets are not bunched or loose before a baby is put in a crib to sleep.
Standard #: 8VAC20-780-350-B-2 Description: Based on observation the center did not maintain the required ratio.
Evidence:
In the 4 year-old room there were 16 children with 1 staff member. The ratio for 4 year-olds is 1 staff for every 10 children.Plan of Correction: The designated person in charge thought because some of the 4 year-old children attend the public re-school program, they met the definition of school-age eligible. This was a misunderstanding of the standards. The director quickly staffed the 4 year-old room appropriately and will review ratios with all staff.
