Comments:
An unannounced monitoring inspection was conducted on 11/28/2023. There were 20 children, ages 17 months - 4 years, and 5 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during arrival, self-directed activities, mealtime, and teacher directed activities. The inspector discussed the following with the provider: medication. A total of 5 children?s records, 5 staff records and 1 medication were reviewed. The inspector arrived for the inspection at approximately 7:05 am and departed at approximately 10:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-60-A
Description: THIRD REPEAT VIOLATION
Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.

Evidence:
1. The record for Child 1 did not contain the following information: work phone number for each parent (an employer was listed but the record did not contain a work phone number) and name address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete).
2. The record for Child 2 did not contain the following information: name and phone number of child?s physician, allergies and intolerance to medication or any other substances and action to take in an emergency situation, chronic physical problems and pertinent development information and any special accommodation needed, previous child day care and schools attended by the child, and name of any additional programs or schools that the child is concurrently attending and the grade or class level.
3. The record for Child 5 did not contain the following information: name address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete), written parent agreements between the parent and the center and name of any additional programs or schools that the child is concurrently attending and the grade or class level.

Plan of Correction: Provider will update forms.

Description: Based on record review and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.

Evidence:
1. The record for Staff 1 did not contain the following: name address and telephone number of person to be notified in an emergency and information about any health problems that my interfere with fulfilling the job responsibilities. Staff 1 confirmed this information was not available at the center, as required by the standard.
2. The record for Staff 2 did not contain the following: name address and telephone number of person to be notified in an emergency and information about any health problems that my interfere with fulfilling the job responsibilities. Staff 1 confirmed this information was not available at the center, as required by the standard.
3. The record for Staff 3 did not contain the following: name address and telephone number of person to be notified in an emergency and information about any health problems that my interfere with fulfilling the job responsibilities. Staff 1 confirmed this information was not available at the center, as required by the standard.
4. The record for Staff 4 did not contain the following: name address and telephone number of person to be notified in an emergency and information about any health problems that my interfere with fulfilling the job responsibilities. Staff 1 confirmed this information was not available at the center, as required by the standard.
5. The record for Staff 5 did not contain the following: name address and telephone number of person to be notified in an emergency and information about any health problems that my interfere with fulfilling the job responsibilities. Staff 1 confirmed this information was not available at the center, as required by the standard.

Plan of Correction: Provider will obtain information from staff.

Standard #: 8VAC20-780-500-A
Description: Based on observations, the center failed to ensure that children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence: Children, ages 2 years- 4 years, were observed participating in self-directed activities and immediately going to tables to eat breakfast. The children did not wash hands with soap and running water or disposable wipes before eating.

Plan of Correction: Staff will ensure children wash hands before eating.

Standard #: 8VAC20-780-560-F-4
Description: Based on observation, the center failed to ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Evidence: Children, ages 2 years- 4 years, were observed were observed during breakfast. The children were served fruit cocktail that contained whole grapes; whole grapes are considered to be potential choking hazards.

Plan of Correction: Staff will remove or cut grapes.
