63.2 CHILD ABUSE AND NEGLECT

Comments:
An unannounced on-site monitoring inspection was conducted on 11/28/2023; a staff record review will be conducted on a later date. There were 71 children, ages 5 years-11 years, and a total of 8 staff members and 1 volunteer present during the inspection. The inspector reviewed compliance in the areas of administration, staffing and supervision, programs, emergencies and nutrition. The children were observed arriving to the program, having an afternoon snack, going outside to play, and participating in homework time and free choice activities. A total of 7 children?s records and 3 medications were reviewed. The inspector arrived to the center at 2:30 pm and departed at 4:10 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-500-A
Description: Based on observations, the center failed to ensure that children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence: The children were observed during the afternoon snack. The children were observed immediately returning to normal activities (going outside to play, homework and free choice activities) after finishing their snack. The children did not wash their hands after eating a snack.

Plan of Correction: Will have staff get children to wash their hands or to use a wipe after snack.

Standard #: 8VAC20-780-510-L
Description: REPEAT VIOLATION

Based on observation and staff interviews, the center failed to ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: Staff 1 verified that there was a medication located in Child 7?s backpack. Medication 1 was in a ziplock bag inside of Child 7?s backpack. The medication was not kept in locked place.

Plan of Correction: Staff will get the medication from the school nurse each day and lock it in a cabinet.

Standard #: 8VAC20-780-510-P
Description: Based on document review and observation, the center failed to ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

Evidence: The authorization for Medication 2 expired on 09/12/2023. Medication 2 was observed on-site during the 11/28/2023 inspection.

Plan of Correction: Will give the mom a new form to get completed.
