The following was discussed with the program director:
8VAC20-780-130. Immunizations for children.
B. The center may allow a child to attend contingent upon a conditional enrollment for a period of 90 days if the child received at least one dose of each of the required vaccines and the child possesses a plan from a physician or local health department for completing his immunization requirements within the ensuing 90 calendar days. If the child requires more than two doses of hepatitis B vaccine, the conditional enrollment period, for hepatitis B vaccine only, shall be 180 calendar days.

A renewal inspection was conducted on 11/27/2023. There were 17 children present and 5 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289-036-B-4 Description: Based on a record review the center did not ensure all required background checks for staff were complete.
Evidence:
Staff #3, hired 09/01/2023, indicated living outside the state of Virginia within the past 5 years. The center did not have a criminal record, sex offender or central registry check from that state for Staff #3.Plan of Correction: The director will send off for the required out-of-state checks this week.
