Technical Assistance:
1. Discussed options for documenting staff training and what types of training count toward the annual requirements.
2. Written injury reports should include the child's first and last name.

Comments:
Conducted an unannounced monitoring inspection at 10:55am. Observed 59 children + 18 direct-care staff. Ratios and supervision were in compliance. Children were observed playing with sensory tables, creative play, reading books, doing puzzles, taking naps and departing for the day. Staff were actively engaged with the children and activities. As this is a language immersion program, most of the conversations between children and staff were in a second language. The school was found to be clean and well supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on records reviewed, 1 of 6 staff records did not include a required out-of-state background check prior to the 1st day of employment. Evidence: Staff #14 began employment in August 2021 and to-date there was no documentation of an out-of-state child abuse and neglect search.

Plan of Correction: Employee #14 has recently submitted a new sworn form, Confirming residency in Virginia for the past five years. [sic]

Standard #: 8VAC20-780-70
Description: Based on records reviewed, 1 of 6 staff records did not include required information. Evidence: The record for Staff #13 did not include documentation of at least 2 reference checks. None were on-file.

Plan of Correction: We've been in contact with Staff #13, and she has submitted the two references as requested.

Standard #: 8VAC20-780-290-A-3
Description: Based on observations made, electrical outlets did not have protective covers in one classroom. Evidence: A power strip had 5 outlets without protective covers in a 4-5 year-old classroom.

Plan of Correction: All five outlets in the classroom have been secured with covers. Our team has thoroughly inspected the entire center to ensure that all outlets are appropriately covered. We have communicated this update to all staff members.

Standard #: 8VAC20-780-510-I
Description: Based on records reviewed, there was no written authorization for 1 of 5 medications on-site. Evidence: A medication belonging to Child #4 did not have a written authorization signed by the parent. Note: This was a long-term medication and also did not have a written authorization signed by the physician.

Plan of Correction: We have engaged with the families, and they have already submitted the medication authorization. They have confirmed their intent to provide the doctor-signed authorization shortly.
