Today an unannounced Renewal Inspection took place between the hours of 1:50pm-3:15pm. There were 34 children with 6 staff within the supervision guidelines. The children were observed playing on the playground, having circle time, being dismissed for the day and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency procedures were observed during this inspection. Standard #: 22.1-289-036-B-4 Description: Based on a review of board member records, 1 out of 3 board members did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Board Member #1 did not have documentation on file of the results of a search of the child abuse and neglect registry or equivalent registry from state #1 and state #2 in which the individual has resided in the preceding five years.
2. Board Member #1 did not have documentation on file of a criminal history record information check for state #2 in which the individual has resided in the preceding five years.
