22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Technical Assistance:
Discussed definition of Preschool and School age groups. Reviewed over updated information that all spring rockers and merry go around cycles need to have the proper fall zone and resilent surfacing.

Comments:
A renewal unannounced mandates monitoring inspection was initiated on November 16, 2023 from 3:19pm - 4:56 pm. There were 38 children with 5 staff present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming,
medication, special care and emergencies and nutrition. A total of four child records and five staff records were reviewed. The children were observed freel yplaying and participating in multiple station activities. The outside area was inspected.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and staff interview, the licensee did not ensure all applicants for employment, employees, applicants to serve as volunteers, and volunteers shall undergo a background check every five years thereafter the initial background check.

Evidence:
Staff #1 most recent fingerprint check was in Feburary 2018 which is more than 5 years old.
Staff #5(applicant) fingerprint and central registry were more than 5 years old confirmed by staff.

Plan of Correction: Staff #1 and #5 went to get updated fingerprint and has been suspended and can return when it comes back in the future a reminder will be set to get the information

Standard #: 8VAC20-780-160-A
Description: REPEAT

Based on record review, the licensee did not ensure each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
Staff #3 hire date 09/14/2022 the TB results were 09/20/2022. Which the date of results were recieved was after the date of employment.

Plan of Correction: In the future a fingerprint will be required before 1st day of work

Standard #: 8VAC20-780-160-C
Description: Based on record review, the licensee did not ensure at least every two years a tuburculosis screening should be completed and obtain results.

Evidence:
Staff #1 most recent TB test provided during the inspection was 07/29/2021.

Plan of Correction: Staff #1 was sent to get a TB screening the next day in the future no one will not work with an updated one

Standard #: 8VAC20-780-80-A
Description: Based on record review, the licensee did not ensure the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
In the school age room, the children were not marked when they departed to school in the AM and when they arrived in the PM. Confirmed with staff.

Plan of Correction: The director had a meeting with all staff to discuss the importance of keeping daily attendance in the future the director will check

Standard #: 8VAC20-780-270-A
Description: Based on observation , the licensee did not ensure the equipment outside was clean, safe and operabelcondition.

Evidence: a steering wheel on the equipment outside was broken and had sharp edges.

Plan of Correction: The equipment on the playground was fixed the same day the staff has been retrained on playground safety
