Provided consultation on the following: 8VAC20-780-160C TB tests are required every 2 years from the last TB test.

Comments:

An unannounced monitoring inspection was conducted on 11/29/2023 from 11:15am to 1:15pm. There were 85 children ages 3 months to age 9 years old supervised by 14 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, 6 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in lunch, reading and nap. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 12/5/23. Description:	Based on review of 5 staff records, the provider did not obtain documentation of the results of a national fingerprint background check prior to date of hire. Evidence: The record for Staff #5 (Date of Hire 8/30/23), did not contain documentation of a fingerprint background check completed prior to their date of hire. The fingerprint background check on file was dated 9/1/23.

Standard #:	8VAC20-780-160-A
Description:	Based on review of 5 staff records, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for staff at the time of employment and prior to contact with children and within 30 days prior to employment. Evidence: 1)The file for Staff #5 (date of hire 8/30/23) did not contain documentation of a negative tuberculosis (TB) test or screening staff at the time of employment and prior to contact with children. The TB test on file was dated 8/31/23.

Description:	Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs. Evidence: During inspection in the Threes classroom, there were 10 children present, but the written record reflected there were 12 children present.
Plan of Correction:	Students signed out immediately after transition.
