1. The area underneath the dome climber must be raked constantly to ensure the mulch remains resilient and if the mulch starts to break down and turn to soil, then you must replace with fresh mulch.
2. On the allergy/sensitivity/dietary restriction list, please list the children's full names and make sure you document the full date the list was last update/revised.
3. Please review the standards regarding medication compliance ( standard 510) and make sure you remain in compliance with those requirements..
4. Please review standards 210 A regarding program lead ( lead teacher) requirement.

Standard #: 8VAC20-780-40-M Description: Based on a review of the allergy/sensitivity/dietary restriction list and interview with staff, it was determined that child A has a Lactose intolerance however that information was not identified on the current written allergy list, which was dated September 2023. Plan of Correction: We will add the sensitivity to the written list.
Standard #: 8VAC20-780-210-A-4-a Description: Based on a review of five staff files and interview with the staff, it was determined that there was no documention of the 24 hours of lead teacher training for 3 staff, who are functioning as lead teachers.
Evidence:
Staff A, B and C are functioning as lead teachers and interviews confirmed such, however there was no 24 hour lead teacher training on file. The 24 hours of lead teacher training is required within one month after being promoted or beginning work as a lead teacher.
Staff A was hired on 9/11/23, staff B was hired on 11/22/21 and staff C was hired on 5/15/23.Plan of Correction: We will either find the documentation for the 24 hours of training or we will provide the training.
Standard #: 8VAC20-780-270-A Description: Based on observation, it was determined that the outside area was not maintained in a safe condition.
Evidence:
The Licensing Inspector observed exposed ground covering around the airplane on the playground. The exposed ground covering presents a tripping hazard.Plan of Correction: We will rake the mulch to ensure the tripping hazard is covered.
Standard #: 8VAC20-780-510-E Description: Repeat Violation:
Base on interview, it was determined that there was no parental authorization for an Inhaler for child A.Plan of Correction: We will obtain written parental authorization.
Standard #: 8VAC20-780-510-H Description: Repeat Violation:
Based on observation and interview, it was determined that an Inhaler for Child A was not in the original container with the pharmacy label attached.Plan of Correction: We will obtain the original prescription box.
Standard #: 8VAC20-780-510-L Description: Based on observation and interview, it was determined that medication ( Inhaler) for child A was not kept locked.
Evidence:
The Licensing Inspector observed the inhaler for child A sitting in a bag on a chair in the office. The medication was unlocked and accessible to the children, who must enter the office to get to their classrooms.Plan of Correction: The medication was locked during the inspection.
