A mandated monitoring inspection was conducted on location on November 27, 2023 from 9:45 A.M.-11:00 A.M, and concluded on November 29, 2023 after completing records review. There were ten children present, ranging in ages from three to five years of age, with two staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, medication, special care and emergencies, transportation, and nutrition. A total of five child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Based on a review of staff records, the center failed to ensure that documentation of a tuberculosis screening shall have been completed within the last 30 days of the date of employment.
Evidence:
1. Staff #1's date of employment was 06/01/2023. The tuberculosis screening documentation in the record was dated 03/29/2023.
2. Staff #3's date of employment was 06/01/2023. The tuberculosis screening documentation in the record was dated 03/17/2023.Plan of Correction: In the future, I will make sure that all TB screenings are completed within 30 days of hire.
Standard #: 8VAC20-780-60-A Description: Based on review of children records, the center failed to ensure that all required information were in the records.
Evidence:
1. Child #1, #3, and #4's record did not contain the date of enrollment.
2. Administration verified that the exact enrollment dates were not listed in the children's records.Plan of Correction: I will make sure that all children's records reflect the exact date of enrollment going forward, and will update the current student records accordingly.
