Comments:
An unannounced on-site monitoring inspection was conducted on 11/16/2023. There were 17 children, ages 5 months-5 years, and 4 staff members present during the inspection. The inspector observed the following: free choice activities, breakfast and morning snack, and TV dance time. A total of 5 children?s records and 2 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 9:15 AM and departed at 11:30 AM.

The list of enrolled children that was sent to the LI on 11/02/2023 per the Special Order was verified and an updated copy of all children enrolled in the program was requested.

Description: Based on review of policies and procedures, the center failed to ensure that the center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Description: 2nd REPEAT VIOLATION

Based on observation, the center failed to ensure that the ratio for infants was met.

Evidence:

Upon arrival at approximately 9:15 AM, the infant room had 6 children 5 months- 16 months of age with one staff member. All 6 children were enrolled into the program and had a record. The required ratio is 4 children to 1 staff person. Two staff members are required for this room. Staff member #5 stated that the fifth child arrived at approximately 8:32 AM. Staff member #2 arrived at approximately 9:32 AM. The infant room was out of ratio from approximately 8:32 AM to approximately 9:32 AM.

Evidence:

Upon arrival at approximately 9:15 AM, the Two-year-old room had 11 children 2 years ? 4 years of age with one staff member. All 9 children were enrolled into the program and
had a record. The required ratio is 8 children to 1 staff person. Two staff members are required for this room. Staff member #4 stated that the nineth child arrived at approximately 9:01 AM. Staff member # 3 arrived at approximately 9:32 AM am. The two year old room was out of ratio from approximately 9:01 AM to approximately 9:32 AM.

Description: Based on observation, the center failed to ensure that all diapering requirements were met.

Description: Based on review of the emergency plan, the center failed to ensure a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and relocation, shelter-in-place, and lockdown. The plan, which shall be developed in consultation with local or state authorities, shall include the most likely to occur emergency scenario or scenarios, including fire, severe storms, loss of utilities, natural
disaster, chemical spills, intruder, violence at or near the center, terrorism specific to
the locality, and other situations, including facility damage that requires evacuation,
lockdown, or shelter-in-place.
