Comments:

An unannounced on-site monitoring inspection was conducted on 11/27/2023. There were 39 children, ages 6 years-12 years, and 10 staff members present during the inspection. The inspector observed the following: arrival into the program, dinner, homework, computer lab and free choice activities. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, lock down drill due before the end of the year. The inspector arrived at 1:55 PM and departed at 5 PM. Description:	Based on review of three staff?s records, the center failed to ensure that all required out of state background checks were obtained. Evidence: Staff # 3 had a documented employment date of 10/09/2023. Staff # 3 indicated on the sworn statement that they have lived outside of the state of Virginia in the past five years. The state on the sworn statement does not participate in the National Fingerprint File which requires a criminal check prior to starting. The CPS check was completed on 5/11/2023

Description:	Systemic Deficiency Based on interview and review of staff records, the center failed to ensure that all staff had a negative tuberculosis (TB) screening at the time of employment, no more than 30 days prior to hire, and before coming in contact with children. Evidence: Staff # 3 had a documented employment date of 10/09/2023 and the date of the TB screening was completed on 06/07/2023.

Evidence: 1.The record for Child # 1 was missing the parents contact phone number. 2. The record for Child # 3 was missing documentation of the father?s work information, allergies and action to take. 3. Child # 5 had a parent for one of the two emergency contacts. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
