Comments:
An unannounced non-mandatory inspection was conducted on 11/13/2023, from 10:05AM to 12:30PM, in response to a complaint received in our office for an incident on 11/07/2023. An investigation was conducted, with another agency, regarding Administration, Staff Qualifications and Training, Staffing and Supervision and Programs. Standard #: 22.1-289.035-B-2
Complaint related: No
Description: Based on interviews, fingerprinting was not completed prior to the first day of service/employment. Evidence: Staff B was used as a substitute for the day and had not completed fingerprinting prior to the day of service/employment.

Plan of Correction: The center will not collaborate with individuals who lack records of their fingerprints from prior employment.

Description: Based on interviews, a Sworn Disclosure Statement had not been completed for Staff B. Evidence: Staff B was used as a substitute for the day and had not completed a Sworn Disclosure Statement prior to the day of service/employment.

Plan of Correction: The center will not commence work with anyone until all required paperwork is obtained.

Description: Based on interviews and record review, a central registry finding was not obtained within 30 days of employment. Evidence: Staff A did not have the results from the central registry finding within 30 days of employment and follow-up was not completed. The date of hire for Staff A was 10/02/2023.

Plan of Correction: The agency returned Staff A's Central Registry Background report on 11/16/2023. The center will address any pending paperwork before the 30th day of employment.

Description: Based on record review, and interviews, tuberculosis screening documentation was not obtained for 2 staff members. Evidence: The center had not obtained tuberculosis screening documentation, before working with children, for Staff A and Staff B .

Plan of Correction: No staff member will have contact with children before obtaining a negative TB test. Staff A underwent a tuberculosis screening on 11/10/2023.

Description: Based on interviews, a complete record was not kept for each staff person. Evidence: Staff B was used as a substitute for the day and did not have a complete file at the center.

Plan of Correction: The center will not engage with anyone who doesn't possess a complete record as mandated by state and county regulations.

Description: Based on interviews, sleep for an infant was not provided, as needed. Evidence: 1. Staff A did not allow an infant to sleep as needed. 2. Staff A said the child's name and gently rubbed/tapped Infant A's chest to wake them up. 3. Staff A rubbed water on the Infant A's face so they wouldn't fall asleep before returning from the playground to the center. 4. Infant A was in a large stroller used for multiple infants.

Plan of Correction: No staff member will withhold sleep under any circumstance, as long as the infant has a safe and appropriate place to sleep.

Description: Based on interviews, rest was withheld from an Infant. Evidence: 1. Staff A attempted to withhold rest from an infant. 2. Staff A gently rubbed/tapped Infant A's chest to wake them up. 3. Staff A rubbed water on Infant A's face so they wouldn't fall back asleep before returning from the playground to the center. 4. Infant A was in a large stroller used for multiple infants.

Plan of Correction: No staff member will withhold rest under any circumstance, as long as infants have a safe place to rest.

Description: Based on interviews and record review, at least one staff in the area where children were present did not have First Aid and CPR training. Evidence: The documentation for First Aid and CPR training showed the training was obtained after the date of the incident for Staff A. The date of the incident was 11/07/2023. The date First Aid and CPR training was completed was 11/11/2023. CPR Staff B did not have any documentation available. Staff A and Staff B were the only two supervising the children on the playground.

Plan of Correction: Staff without First Aid and CPR training will not supervise children independently; they will always be accompanied by staff with the necessary training.
