Technical Assistance:
Technical Assistance given and licensing standards regarding diapering, resilient surfacing, and staffing were discussed.

Comments:
An unannounced monitoring inspection was conducted on October 25 and 26th, 2023. The inspectors arrived at 12:30pm and departed at approximately 3:40pm. There were 111 children present and 20 staff members. A sample size of 18 records were reviewed which included 10 children?s records and 8 staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during group activity, snack time, lunch, handwashing, and naptime. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director and owner at the exit interviews.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview it was determined that the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening that has been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. Three staff members have their documentation of their tuberculosis (TB) screening dated prior to last 30 calendar days of the date of employment:
a. Staff #2 (date of hire 7/24/23) has a TB screening dated 3/21/2023;
b. Staff #3 (date of hire 9/5/23) has a TB screening dated 2/19/2023;
c. Staff #4 (date of hire 10/23/23) has a TB screening dated 9/10/2023.
2. Staff #1, director, confirmed that Staff #2, Staff #3, and Staff #4 have TB screenings prior to the last 30 calendar days of the date of employment.

Plan of Correction: The center responded with the following: Moving forward, the center will ensure that TB screenings for new staff are completed timely.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the 2 year old classroom bathroom, there was a bottle of air freshener that has a warning label, ?Keep out of reach of children,? that was unlocked.
2. 2 year old staff members confirmed that the bottle of air freshener was unlocked.
3. Staff #1 confirmed that the bottle was unlocked.

Plan of Correction: The center responded with following: The center will ensure that all bottles of cleaner and sprays will be locked and away from children.

Standard #: 8VAC20-780-350-A
Description: Based on observation and interview, it was determined that the center did not ensure that the maximum group size limitations for children 2 year old is a maximum group size of 24 children.
1. In the 2 year old classroom, there were 32 children and 4 staff members in one room. The lead teacher was in charge of all 32 children in the classroom and functioned as one large group.
2. All 2 year old staff members confirmed that the classroom with 32 children is one large group and over the maximum size of 24 children per grouping.

Plan of Correction: The center responded with the following: The 2 year old classroom used to group the children in the one classroom however it was too chaotic and the system did not work consistently. The center will look at restructuring the classroom as there they are over the maximum group size requirements for 2 year old children. The center will let DOE know the plans regarding maximum group sizing to ensure that it meets the Virginia Standards.

Standard #: 8VAC20-780-440-G
Description: Based on observation and interview, it was determined that the center did not ensure that cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat.
Evidence:
1. In the infant room, there were two children ages, 13 months, Child #1 and Child #2, that were observed sleeping in cribs.
2. Infant staff confirmed that Child #1 and Child #2 are sleeping in cribs and are over the age of 12 months.
3. Staff #1 confirmed that the children are over 12 months of age and are sleeping in cribs.

Plan of Correction: The center responded with the following: The center will ensure that when children are turning 12 months of age, they will transition from the crib to a mat or cot.
