Technical Assistance:
As discussed please ensure your crib sheets are tight fitting to prevent material from gathering or prohibit the mattress from lying flat.

Also per our discussion, please remove any stuffed toys from the ends of pacifiers while infants are in cribs.

Comments:
An unannounced Monitoring Inspection was conducted this date. A total of 113 children with 23 staff were onsite during the inspection. Walk through of both buildings was conducted today. Observation of the infants who were on the floor playing and interacting with staff was made. Cribs were inspected. In the toddler room, staff were observed sitting in a group along with the children while one staff read a book. Topical ointments were onsite and reviewed. Handwashing procedures were observed as the children began transitioning to lunch. Lunch today was chicken, mashed potatoes, fruit salad with milk to drink. The PreK class was observed sitting on an area carpet listening to the lead teacher allowing the children to interact and ask questions in between the book discussion. Holiday decorations were in progress in each classroom. A total of 8 children's records which included medication authorizations were reviewed along with 4 staff records. The previous violations from the last inspection were reviewed and corrections were verified. This inspection occurred between 10:15am and 1:20pm. Thank you for your cooperation today. Standard #: 8VAC20-780-510-E
Description: Based on review of medications, the center failed to follow their own procedures for administering medications.
Evidence:
1. Medication for Child #1 expired in September 2023.
2. Medication for Child #2 expired in June 2023.
3. Medication for Child #3 expired in August 2023.
4. The physician authorizations for all 3 long term medications had expired.

Plan of Correction: All expired medications will be either disposed or returned to the parent.

Standard #: 8VAC20-780-510-I
Description: Based on review of medication, an expired medication was administered to a child without a current parent/physician authorization.
1. Child #2 was administered a medication on 10/16/2023.
2. The medication expired in June 2023.
3. The parent/physician authorization expired in July 2023.

Plan of Correction: All new authorizations for expired medication will be obtained. No expired medications will be administered.
