Comments:
An unannounced renewal inspection was conducted today from 9:40am-12:00pm. There were 83 children directly supervised by 24 staff. The physical plant, 7 staff records, 11 children?s records, 4 renewal applicant background medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to an audio story book, playing outside, and eating snack, and dancing. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Evidence:
Applicant 4's record did not contain the results of a national criminal background check.

Plan of Correction: Got corporate officer's background check processed.

Standard #: 8VAC20-770-40-D-1-a
Description: Based on record review, the licensee failed to obtain the renewal applicant's sworn statement or affirmation or the search of Virginia's central registry.
Evidence:
Applicant 4's record did not contain the results of a central registry background check for Virginia, or a sworn statement or affirmation by the applicant.

Plan of Correction: Got corporate officer's central registry and sworn statement processed.

Description: Based on record review, the center failed to meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.
Evidence:
The record for Child 7 did not contain documentation that the child's age and identity had been verified by the child day center.

Plan of Correction: Got Birth Certificate from parent.

Description: Based on a review of medication, the center
did not follow its procedures to administer
medication.
Evidence:
Child 11 has a written authorization to
administer Diphenhydramine, however the
medication present for the child on the date of
inspection was Diphenhydramine and
Phenylephrine.

Plan of Correction: Got new medication from parent.
