Comments:
A renewal inspection was conducted today between 9:00am and 12:00pm. There were 66 children present, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed playing outside, making ornaments, story time, playing with table toys, and getting ready for lunch. A total of 6 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, not all staff obtained documentation of a tuberculosis (TB) test within 30 days prior to employment.

Plan of Correction: Going forward I will make sure the TB tests are not more than 30 days before their first day of employment. I misunderstood the standard.

Standard #: 8VAC20-780-40-G
Description: Based on a review of children's records, the center did not meet the proof of child identity and age requirements.

Plan of Correction: I will get the proof of identity from the parent.

Standard #: 8VAC20-780-280-B
Description: Based on an observation of the center, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: All chemicals were removed during the inspection. I will make sure all staff understand all chemicals need to inaccessible to the children.

Standard #: 8VAC20-780-520-A
Description: Based on observation of the center, nonprescription drugs and over-the-counter skin products were not used in accordance with the manufacturer's recommendations. Over-the-counter medications were kept beyond the expiration date of the product.

Plan of Correction: The staff will be retrained to check all topicals monthly to ensure they are not expired.

Standard #: 8VAC20-780-520-C
Description: Based upon an observation of the center, diaper ointment or cream was not kept in a location inaccessible to children.

Plan of Correction: All staff will be retrained to ensure all topicals are in accessible to the children.

Standard #: 8VAC20-780-550-P
Description: Based upon a review of records for children's serious and minor injuries, there was no documentation of future actions to prevent recurrence of the injury, or documentation of how the parent was notified.

Plan of Correction: We will begin immediately using the correct incident/accident report that includes all the required information.

Standard #: 8VAC20-780-560-G
Description: Based on an observation of the center, food containers from home were not clearly dated and labeled in a way that identifies the owner.

Plan of Correction: Staff will be retrained to ensure all food brought from home is labeled with the current date and name.
