Comments:
A renewal inspection was initiated and concluded on 12/5/2023.There were 25 children present with 5 staff supervising and 1 administrator. The children were observed during classroom time engaged in lessons and observed during outside play time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:15 am and concluded at 12:05 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review and staff interview the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: The director confirmed that Child #3 (start date 8/24/2023) did not have documentation of immunizations.

Plan of Correction: Make sure to have by 30 days. Received the immunization by fax 12/5/2023.

Standard #: 8VAC20-780-140-A
Description: Based on record review and staff interview the center did not ensure each child shall have a physical examination before the child?s attendance; or within 30 days after the first day of attendance.
Evidence: The director confirmed that Child #3 (start date 8/24/2023) did not have documentation of a physical examination.

Plan of Correction: Make sure to have by 30 days. Received the physical form by fax 12/5/2023.

Standard #: 8VAC20-780-280-B
Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: The director confirmed in the following rooms hazardous chemicals were observed in an unlocked manner and they were labeled "keep out of reach of children" and at least one other statement "caution", "flammable" and "warning".
1. In the Preschool kitchen / lunchroom there was a bottle of Meyers hand soap on the counter by the sink.
2. In the boy?s bathroom there was a hand soap dispenser on the counter by the sink. The owner confirmed it was filled with soft soap.
3. In the School Age / Infant kitchen there was a bottle of Meyers dish soap on the counter by the sink.

Plan of Correction: Changed out the hand soaps to soft soap which does not have to be out of reach.

Standard #: 8VAC20-780-550-D
Description: Based on record review and staff interview the center did not implement a monthly practice evacuation drill as required.
Evidence: The director confirmed the center's evacuation drill sheet did not include documentation of an evacuation drill in the month of November 2023.

Plan of Correction: Make sure we do a drill each month.
