A complaint inspection was conducted on 12/07/2023. The inspector arrived at 12:45 PM and departed at 1:25 PM. There were 9 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration and supervision. The complaint investigation included interviews and review of documentation.
Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard #: 8VAC20-780-40-E Complaint related: Yes Description: Based upon review of parent policies and staff interview, the facility has not ensured that activities and services are maintained I ncompliance with the center's own policies and procedures required by the standards.
Evidence:
1. The facility's written procedure provided to parents for action that will be taken if a child is not picked up after the center is closed, stated that child protective services will be contacted.
2. Staff 1 confirmed the written policy and stated that on November 24th, when child 1 was not picked up after the facility closed and the parent could not be contacted, child protective services was not contacted.Plan of Correction: The facility responded with the following:
Director also advanake information in policy stating in parent is listed on birth certificate they need to be put on the pick up list if there is no custody order.
Parents are also inform of changes made to the policy if child is not pick up by closing.
Center will be following the policy step by step.
Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Based upon interviews and review of documents, the facility has not ensured that when staff are supervising children, they always ensure their protection.
Evidence:
1. Staff 1 stated that on November 22, 2023, when the parent could not be reached to pick up child 1, 2 1/2 years old, after the facility closed, she contacted a relative and released child 1 to the relative who was not authorized by the parent to pick up the child.
2. A review of the child's record confirmed that the parent had not authorized anyone other than herself to pick up child 1.Plan of Correction: The facility responded with the following:
Director had asked parent to list all and more than one person other than parent to pick up.
