An unannounced non-mandated monitoring inspection was initiated on November 30, 2023 and concluded December 8, 2023. The non-mandated monitoring inspection was in response to a self-reported incident regarding allegations in program director qualifications. Interviews were conducted.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the self-reported incident but identified during the course of the investigation can be found on the violation notice.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1)
steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-200-A Description: Based on observation and interview, the center failed to have a qualified program director or a qualified back-up program director who meets one of the director qualifications who shall regularly be on site at least 50% of the center's hours of operation.
Evidence: 1. During an inspection on November 16, 2023, staff #1 was observed acting as, and stated that she is the program director of the center.
2. The record for staff #1 confirmed that she was designated as the program director on October 17, 2023 but the record did not include documentation of program director qualifications.
3. In an interview with administration on November 30, 2023, it was stated that staff #1 doesn?t meet program director qualifications. Administration stated that the center did not have a qualified program director from October 17, 2023 to November 27, 2023.Plan of Correction: Not available online.
