1) 80: Attendance that documents the arrival and departure of children in care shall be accessible to emergency responders, and not rely on an electronic system that requires the internet, power, or a code to obtain access as this isn?t reliable during an emergency
2) 440L: Pillows shall not be used by children under two years of age; a blanket with attached pillow is not allowed for infants, toddlers and two year old children
3) 500B: The diapering area shall have the following: a non-absorbent surface for diapering. For children younger than three years, this surface shall be a changing table or countertop designated for changing (not standing up in the bathroom); a leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal; ensure that a containers are accessible, operable, and do not require the staff to touch another surface during disposal; a leakproof covered receptacle for soiled linens

Comments:
An unannounced monitoring inspection was conducted from 1:30 - 4:30 pm with the director. There were 87 children, ranging from eight months to five years of age, present with 13 staff supervising. Infant care was observed and older children were observed napping, waking, and preparing for snack. Snack served today: sun butter and apples. Eight child records and seven staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, six medications and their authorization forms and allergy care plans. The playground area was also observed.

Description: (Repeat Violation) Based on review of seven staff records, it was determined that no Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment and no Sex Offender Registry search was conducted prior to employment for staff who indicated they have lived outside of Virginia in the past five years. Evidence: the record for staff #3 (Date of hire: 10/10/2023), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state child abuse and neglect search request, or a sex offender registry search.

Description: (Repeat Violation) Based on review of seven staff records, it was determined that not all staff had documentation of a negative tuberculosis (TB) test/screening completed within the last 30 calendar days of the date of employment. Evidence: the record for staff #1 (date of hire: 6/20/2023) had documentation of a negative TB test/screening dated: 1/5/2023; the record for staff #2 (date of hire: 9/20/2023) did not have documentation of a negative TB test/screening; the record for staff #7 (date of hire: 8/15/2022 had documentation of a negative TB test/screening dated: 4/30/2023.

Description: Based on observation, it was determined that not all areas inside the center were maintained in a safe and operable condition. Evidence: In the infant A classroom, there were two cribs with loose fitting sheets and in the infant B classroom, there were three cribs with loose fitting sheets on the mattress. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.

Description: Based on observation, not all handwashing procedures were followed, as required. Evidence: in the infant classroom, one child was observed drinking a bottle and their hands were not washed with soap and water beforehand, and another child did not have their hands washed after a diaper change.

Description: Based on review of medication, it was determined that not all medication was labeled with the child?s name, the dosage amount and the time to be given. Evidence: the medication for child #2 was not in it?s original package and did not have a medication label that included the child?s name, dosage amount and the time to be given.

Description: (Repeat Violation) Based on review of medication and medication authorization forms, the facility did not ensure that when an authorization for medication expired, parents were notified to pick up the medication within 14 days or renew the authorization form. Medications that aren?t picked up by the parent within 14 days must be disposed of. Evidence: the record for child #9 had an authorization form that expired on 6/30/23; the record for child #10 had an authorization form that expired on 9/14/23. Both medications were observed on site.

Description: Based on observation, and a review of documentation, the facility failed to ensure that at least one staff in each classroom has current certification in CPR/first aid. Evidence: Staff #2, #3, #4, #5 and #6 were all observed working alone in a classroom; the record for staff #2 did not have documentation of CPR/first aid, the record for staff #3, #4 and #6 did not have documentation from an approved CPR/first aid program that included an in-person component, and the record for staff #5 had documentation of CPR/first aid that was dated 2/17/21 and is expired.
