Comments:
An unannounced inspection was conducted in response to a self reported incident that occurred on 11/2/23 in the Toddlers/Two's classroom. The incident was related to a child that was left unsupervised in the classroom. 3 staff members were interviewed, 1 staff record and 1 child's record was reviewed. Child supervision records and the center's policies were also reviewed. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names, list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Standard #: 8VAC20-780-40-E
Description: REPEAT VIOLATION:
Based on records review and staff interview, the center did not follow its own policies and procedures that are required by the standards.
Evidence:
1. The center?s ?Supervision of Children? policy states, ?Enrolled children must be supervised at all times. This means that staff members must never leave a child or children unattended for even a moment?. The policy also states that, ?Leaving a child in the classroom while others move to another room are all examples of lack of supervision?.
2. The center?s ?Child Supervision Records? (CSR) policy states that ?Name to face attendance (visually confirming children are present by matching the names on the CSR to the faces in the classroom) must be taken every 30 minutes and during any large class transitions (i.e., going outside to the playground).
3. On 11/2/23, at approximately 10:10 am, Staff # 1 and Staff # 2 left Child # 1 (1 year old) unattended in the classroom for approximately 2 minutes when they left the room with 7 other children to go to the playground.
4. Staff # 1 stated that at approximately 10:10 am, they were lining up to go outside for the fall festival and were running behind. Staff # 1 stated the children were not lined up, and she did not do a name to face count using the CSR. Staff # 1 thinks that Child # 1 was unattended for approximately two minutes from the time they went outside.
5. Staff # 2 stated the children were lined up to go outside, and Staff # 2 did not do a name to face count using the CSR. Staff # 2 stated that Child # 1 was unattended for approximately two minutes.

Plan of Correction: Staff has been retrained on the proper use of CSR's. Management committed to ensuring that all staff keeps close supervision of children.

Standard #: 8VAC20-780-340-F
Description: Based on staff interviews, children under 10 years of age were not always within sight and sound supervision of staff.
Evidence:
1. On 11/2/23, at approximately 10:10 am, Staff # 1 and Staff # 2 left Child # 1 (1 year old) unattended in the classroom for approximately 2 minutes, when they went to the playground.
2. Staff # 3 stated that at approximately 10 am when she was checking classrooms, she stepped into the large room next to the Toddler room and saw Child # 1 standing alone in the classroom.
3. Staff # 3 stated that she quickly took Child # 1 outside and gave the child to Staff # 1. Staff # 3 stated that both staff had just stepped outside with the rest of the children, using the side door that leads around to the playground.
4. Staff # 1 stated that at approximately 10:10 am Staff # 1 and Staff # 2 were running late to go outside. Staff # 1 stated that she did not do a name to face count of the children before exiting the classroom.
5. Staff # 1 stated that Staff # 3 brought Child # 1 outside to her.
6. Staff # 2 stated that she did not do a name to face count of the children before leaving the room to go outside.
7. Staff # 2 stated that when Staff # 3 brought Child # 1 outside to Staff # 1, she noticed that Child # 1 was crying.
8. Staff # 1 and Staff # 2 stated that Child # 1 was left unattended for approximately 2 minutes.

Plan of Correction: Staff will have continual training on the proper use of CSR's and using the name to face recognition so that children are not left behind or unsupervised. Staff have been notified that children should always be in sight and sound of teachers.

Standard #: 8VAC20-780-440-E
Description: Based on observation, occupied cots were not 12 inches apart.
Evidence: At least 7 occupied cots in the 3?4-year-old classroom with 19 children were not spaced at least 12 inches apart.

Plan of Correction: The cots are now 12 inches apart from each other.
