Today an unannounced Monitoring Inspection took place between the hours of 9:45am and 10:50am. There was 1 classrooms observed with a total of 11 children with 2 staff within the supervision guidelines. The children were observed arriving for the day, changing into their school shoes and washing hands, playing in small group centers while their classmates arrived and circle time. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Positive interactions between children and staff were observed. Violations were found during todays inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-780-240-E Description: Repeat Violation.
Based on record review and interview, each staff has not completed orientation training in first aid and cardiopulmonary resuscitation (CPR) within the first 30 days of employment.
Evidence: Staff #5 (date of hire: 10/15/2022) record does not contain documentation of CPR/first aid orientation. Program management confirmed that the staff had not completed the training.Plan of Correction: If this can be done quickly online, we will get it done in two weeks.

Based on record review and interview, there was not at least two staff members with current first aid and cardiopulmonary resuscitation (CPR) present on the premises during the center's hours of operation.
Evidence:
1. Staff #1's most recent CPR/first aid training expired 12/10/2022.
3. Staff #5 has not yet completed CPR/first aid training.
4. Staff #1 confirmed that staff #2 is the only individual in the program with current CPR/first aid training.Plan of Correction: We thought we only needed on staff training. We will have our second classroom teacher complete the training.
