Technical Assistance:
Technical Assistance was provided to the director regarding staff records and background checks, shelter-in-place drills, and documentation for child records.

Comments:
An unannounced monitoring inspection was conducted on 12/11/2023 from 10:15am to 12:40pm with the program director. There were 54 children in care, ranging in age from two-years to five-years-old, supervised by 10 staff. The children were observed listening to teachers read stories, playing in structured centers, and eating snack from home. Five child records and four new staff records were reviewed. The Center has multiple staff with current certification in CPR and First Aid as well as trained in Daily Health Observation and Medication Administration Training. Three medications and authorization forms were reviewed. Required postings were observed. The attendance and emergency drill log were reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 12/18/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.035-B-2
Description: REPEAT Based on review of four staff records, the program did not obtain documentation of the results of a national fingerprint-based criminal background check prior to employment for one staff. Evidence: The fingerprint results on record for Staff #3, with an employment date of 9/13/23, were dated 9/19/23.

Plan of Correction: Plan to improve the hiring process to be sure that checks are done in timely fashion.

Standard #: 8VAC20-770-60-B
Description: Based on review of four staff records, the program did not obtain documentation of a completed sworn disclosure statement for each staff prior to their date of employment. Evidence: The sworn disclosure on record for Staff #3, with an employment date of 9/13/23, was dated 9/15/23.

Plan of Correction: Plan to improve the hiring process so sworn disclosures completed on or before employment date.

Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records, the program did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The TB screening on record for Staff #1, with an employment date of 8/24/23, was dated 9/1/23. The TB screening on record for Staff #2, with an employment date of 8/30/23, was dated 9/2/23. The TB screening on record for Staff #3, with an employment date of 9/13/23, was dated 9/20/23.

Plan of Correction: Plan to have TB screen results completed prior to first day of employment.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the program did not ensure that medication, except for those prescriptions designated otherwise by written physician's order, was kept in a locked place using a safe locking method that prevented access by children. Evidence: On 12/11/23 one two-year-old child?s emergency medication was observed in a pouch inside of a bookbag, hanging by the classroom door.

Plan of Correction: Corrected during the inspection. Moving forward plan is to lock meds in a cabinet until locking bags arrive.
