Technical Assistance:

Discussed the importance of monitoring children while they are eating to ensure their safety and monitor their risk of choking.

Comments:

An unannounced Monitoring inspection was conducted today. There were a total of 88 children with 14 staff onsite. A total of 7 occupied classrooms were observed today. The children were transitioning from morning activities to preparing for lunch. Handwashing and diapering procedures were observed. In some classrooms the children were sitting in a circle while staff read books such as "Messy Room" and "Holiday in Sweden". The infants were observed eating or being held by staff. One infant was observed sleeping in a crib. Some medication was onsite, and was reviewed along with medication authorizations. Classroom #3 was determined a balanced mixed age grouping of 3,4 and 5 year olds. A total of 4 children's files and 5staff files were reviewed today. This inspection occurred between 11am and 2pm. Standard #:	22.1-289.035-B-2
Description:	Based on review of staff records, background checks were not obtained for 1 staff. Evidence: Staff #4 was hired on 02/21/2022. A fingerprint background check was not requested prior to hire.
Plan of Correction:	Fingerprint appointment has been made and scheduled for tomorrow the 19th of December.
Standard #:	8VAC20-770-60-C-2
Description:	Based on staff record review, background checks were not obtained within 30 days of employment. Evidence: Staff #4 was hired on 02/21/2022. Results from a Central Registry search was not found.
Plan of Correction:	A background check has been submitted in the portal, waiting for results.
Standard #:	8VAC20-780-340-F
Description:	Based on observation, children under the age of 10 years were not within actual sight supervision of staff. Evidence: 1. There were a total of 4 children in Classroom #6 with 1 staff. 2. One of the four children were in the bathroom with the 1 staff. 3. The remaining 3 children were in the classroom and were not within actual sight of staff.
Plan of Correction:	Making sure for the toddler community that there are 2 staff during the most important times of transition. This will make sure that sight and sound of children in the bathroom or some students already on their cots that they are within supervision

Description:	Based on observation of one classroom, staff failed to sit with children while eating. Evidence: 1. In Classroom #5, children were seated at tables eating lunch. 2. The 2 staff in the classroom were not sitting with the children while they ate.
Plan of Correction:	Spoke with the guides in classroom 5 to make sure they are sitting with the children when they are eating
