Comments:

A monitoring inspection was conducted on November 29, 2023 with the center staff. There were 67 children present, ranging in ages from 4 years to 12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.035-A
Description:	Based on review of five staff records, the facility failed to have employees complete background checks in accordance with the Code of Virginia prior to employment and every five years. Evidence: 1. The record for Staff 2 contained the results of a fingerprint national criminal record check dated 05/18/2018. Management stated a repeat check had not been conducted. 2. The record for Staff 3 contained the results of a fingerprint national criminal record check dated 08/06/2018. Management stated a repeat check had not been conducted. 3. The record for Driver 1 (date of employment 9/03/2019) and Driver 2 (date of employment 6/05/2023) did not have results of a fingerprint national criminal record check on file.
Plan of Correction:	Staff will schedule fingerprint appointments as soon as possible.
Standard #:	8VAC20-770-60-B
Description:	Based on review of five staff records, the facility failed to have documentation of a completed sworn disclosure statement on file by the first day of employment. Evidence: Driver 1 (date of employment 9/3/2019) and Driver 2 (date of employment 6/05/2023) did not have a completed sworn disclosure statement on file. Staff 3 (date of employment 7/17/2023) did not complete a sworn disclosure statement until 9/29/2023.
Plan of Correction:	All staff will complete a sworn disclosure statement prior to employment and every five years as required.
Standard #:	8VAC20-770-60-C-2
Description:	Based on review of five staff records, the facility failed to have documentation of a completed search of the central registry (CPS check) on file for each staff within the first 30 days of employment. Evidence: Staff 3 (date of employment 7/17/2023), Driver 1 (date of employment 09/03/2019), and Driver 2 (date of employment 6/05/2023) did not have a completed search of the central registry on file. There was no documentation of having submitted the search of the central registry within 7 days of employment as required.
Plan of Correction:	Management is having difficulty with the search of the central registry portal.

Description:	Based on observation, the facility failed to maintain the required ratio of 1 staff for every ten children age 3 years old up to school age. Evidence: On 11/29/2023, there were 13 preschool children alone with 1 staff.
Plan of Correction:	Staff will maintain appropriate ratios in each classroom.
Standard #:	8VAC20-780-530-A-1
Description:	Based on staff interview and record review, the facility failed to have one staff in each grouping of children with cardiopulmonary resuscitation (CPR). Evidence: Driver 1 and Driver 2 are alone with children on each bus run, but do not have documentation of current CPR on file. Management stated the drivers do not have CPR certification.
Plan of Correction:	All staff will get first aid and CPR before being alone in a grouping of children.

Description:	Based on staff interview and record review, the facility failed to have one staff in each grouping of children with first aid certification. Evidence: Driver 1 and Driver 2 are alone with children on each bus run, but do not have documentation of current first aid certification on file. Staff confirmed that the drivers do not have first aid certification.
