Comments:
An unannounced monitoring inspection was conducted on Tuesday, December 12, 2023, from 10:10am until 12:45pm. There were 47 children present with 6 staff supervising. The inspector observed children playing in centers and interacting appropriately with staff, participating in circle time, participating in a music and movement activity, and listening to a story. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and interview, the center failed to ensure that all employees shall undergo background checks every five years.

Evidence:
1. The staff record for Staff #5 contained a central registry background dated 7/10/2018. Staff #4 should have undergone a criminal background on or before 7/10/2023.
2. An administrator verified that an updated central registry background had been completed for staff #4 but could not locate the central registry background results.

Plan of Correction: The Executive Assistant will get staff to complete required application by end of the day. The executive assistant will ensure that all documents are collected and maintained according to standards.

Standard #: 8VAC20-780-40-M
Description: Based on observation, the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities and dietary restrictions.

Evidence:
The Head Start 3?s and Head Start 4?s classroos, had a list of children?s allergies, sensitivities, and dietary restrictions posted, but the list was not covered.

Plan of Correction: The social worker will place covers labeled "Allergy Health" over all allergy lists by the end of the day.

Standard #: 8VAC20-780-60-A
Description: Based on review of child's record, the center did not ensure to obtain all required information.

Evidence:
The record for child #4 did not contain, proof of the child's age and identity, doctor?s contact information, and 2 emergency contacts did not contain addresses.

Plan of Correction: The social worker will meet with mom to get updated and complete contact information within 24 hours.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation and interview, the center failed to ensure that staff-to-child ratios were maintained.

Evidence:
1. Upon entering the Head Start 4?s classroom, the licensing inspector observed 13 children (ranging from 3 years old to school age) with 1 staff. The required ration is 1:10.
2. In the Head Start 3?s classroom, the licensing inspector observed 12 children (ranging from 3 years to school age) with 1 staff. The ration is 1:10.
3. Program Director acknowledged the classroom were out of ratio.

Plan of Correction: Director will meet with teachers to discuss/remind them that they must notify the lead teacher when out of ratio. Especially sine there were enough staff present to provide adequate coverage.
