An unannounced complaint investigation was initiated along with Licensing Inspector Keesha Minor on 10/18/23 in response to a complaint received in the licensing office regarding allegations of forbidden actions. Based on a review of available documentation and interviews conducted, the allegation regarding forbidden actions was unable to be substantiated due to insufficient corroborating evidence. Based on a review of five staff records, the center did not obtain all required documentation for staff records.
Evidence:
1. The record for Staff #2 did not contain documentation of an emergency contact.

Based on video footage, the center did not ensure behavioral guidance was constructive in nature, age, and stage appropriate.
Evidence: On 10/04/23 in the three-year-class, Staff #2 was observed picking up Child #1, removing them from the circle time area, placing them in a chair at an empty table and walking away. Child #1 slipped out of the chair and was observed walking around crying for approximately four minutes. Child #1, still crying, walked up to Staff #1 sitting on a table at the front of the classroom, Staff #1 leaned forward, placed their fingertips on Child #1?s chest and pushed them back.Plan of Correction: Not available online.
