An unannounced inspection was initiated on 11/30/2023 and concluded on 12/14/2023 in response to a self report that was received by the department on 11/21/2023 regarding allegations in the area of termination policies. The inspection was initiated on site on 11/30/2023. The inspector was on site from approximately 9:00 am-11:00 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the self-report is determined to be not valid. There are no violations resulting from this inspection.
