An unannounced monitoring inspection was conducted on 12/7/2023 from 12:00 pm to 2:10 pm There were 94 children (infants-5yrs.) supervised by 16 direct care staff. The physical plant, 5 staff records, 8 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Please submit your plan of correction within 5 business days for it to be included on the website.

Evidence:
1) There was a separation in the seam of the resilient surfacing near the yellow bars, treehouse, and seesaw, causing a tripping hazard on the playground.
