Comments:
A monitoring inspection was conducted on 12/05/2023 between the hours of approximately 11:00 am and 12:10 pm. There were 15 children present, with 4 staff supervising. Children and staff were observed while coloring and playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on record review, it was determined that the center did not ensure that Central Registry search results were obtained by the end of the 30th day of employment.
Evidence:
1) On the date of inspection, the record for Staff #4 (start date 09/21/2023) did not contain the results of a Central Registry search request. A search request was requested but no follow up was documented in the record.

Plan of Correction: A new form was sent. Waiting for an answer.

Standard #: 8VAC20-780-140-A
Description: Based on record review, the center did not ensure that each child have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1) The record for Child #1 (start date July 2022) did not contain the results of a physical examination.

Plan of Correction: Gave the parent a new physical form to take to the doctor.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center did not ensure that employees obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years.
Evidence:
1) The Tb screening form in the record for Staff #2 was dated 09/12/2021.

Plan of Correction: Staff was told to follow up with the Tb screening.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the first date of attendance.
2) The record for Child #2 did not contain the work phone number and place of employment of a second parent who has custody.

Plan of Correction: Child #1 - Filled out the information that was missing. Child #2 - Informed the parent and monitor the parent while he/she fills in the missing information.

Standard #: 8VAC20-780-420-E-3
Description: Based on record review, the center did not ensure to annually, review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 07/13/2022.

Plan of Correction: Will inform the parent that an update is needed and will monitor the parent while he/she fills the updated form.
