Technical Assistance:
Consultation was given on completing the required VA Preservice training for newly hired staff within 90 days of employment, DOE model forms on the childcareva.com website and training hours such as MAT, first aid and CPR counting towards the 16.0 annual hours needed each year for staff.

Comments:
An unannounced monitoring visit was conducted. 28 children were present with 10 direct care staff and in ratio for supervision. Observed children in 4 classrooms transitioning to the playground, free play, Montessori work in centers (numbers, painting and coloring, letters and circle time). Emergency medication was reviewed for 3 children with Medication Administration Training (MAT) certified staff on-site. Lunch to be served today was spinach manicotti with fruit, winter veggies and whole wheat bread. Bottles/ food from home were labeled with the date and names of children. Emergency preparation is current for drills, supplies and trainings. The Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-245-C
Description: Based on orientation documentation, the form used to orient Staff #1 was incomplete.
Evidence:
The orientation form was missing safe sleep and SIDS syndrome, prevention of shaken baby syndrome and abusive head trauma as well as prevention and response to food and other allergic reactions.

Plan of Correction: The updated DOE orientation model form will be used moving forward. Staff #1 will be re-oriented to all missing components and the orientation form placed in the personnel file. ,

Standard #: 8VAC20-780-550-P
Description: Based on record review, a sample of injury forms were incomplete.
Evidence:
6 injury forms were missing the time the parent was notified and 1 was missing "future action to prevent re-occurrence"

Plan of Correction: Staff will be reminded to complete all lines on the injury form and reviewed by an administrator.
