Comments:
A monitoring inspection was conducted on 12/14/23 from 10:35am until 12:15pm. At the time of the inspection, there were 16 children in care with 5 staff present. A sample of 5 children's records and 4 staff records were reviewed. Children were observed participating in learning activities and an arts and crafts project, eating lunch and resting quietly during nap time. Handwashing and restroom procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and required center postings were reviewed. The center is not currently administering medication. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program and discussed with the center director during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening that is submitted at the time of employment and prior to coming in contact with children.

Evidence:
1. The record for staff 1 does not include documentation of a negative tuberculosis screening.
Staff 1 was caring for children on the day of the inspection.
2. The center director confirmed that the record for staff 1 is lacking documentation of a negative tuberculosis screening.

Plan of Correction: The center responded with the following: Staff 1 will obtain a TB screening as soon as possible. The results will be placed in staff 1's record.

Standard #: 8VAC20-780-60-A
Description: Based on record review and interview, the center did not ensure that children's records contain all the required information.

Evidence:
1. The records for child 1 (date of enrollment: 9/9/23), child 2 (date of enrollment: 9/9/23) and child 3 (date of enrollment: 9/4/23) do not contain the work phone number and place of employment of each parent.
2. The record for child child 4 (date of enrollment: 9/18/23) does not include the work number for the father.
3. The record for child 5 (date of enrollment: 9/1/23) does not include the work number for either parent.
4. The center director confirmed that the above listed information is missing from the children's records.

Plan of Correction: The center responded with the following: The missing work information for the parents of each child will be obtained and placed in the record.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the infant classroom, there was a spray bottle of bleach water and a tub of disinfecting wipes on an open shelf.
2. In the toddler classroom, there was a tub of disinfecting wipes on an open shelf.
3. In the preschool classroom, there was a spray bottle of bleach water, a tub of disinfecting wipes, a can of Lysol and a can of air freshener on an open shelf.

Plan of Correction: The center responded with the following: The unlocked cleaning materials were locked up during the inspection. All hazardous materials will be kept locked in the office until cabinets that can be locked are purchased for each classroom.
