Standard #: 8VAC20-780-340-F
Complaint related: No
Description: Based on staff interview and review of the center's own video recording, the center did not ensure that children under 10 years of age were always within sight and sound supervision of staff.

Evidence: On October 25, 2023 Child #1 (Age 2) was left on the playground after the Threes room went back inside after their outside play time. Staff #1 took the other children in the Threes room back into the center at approximately 3:29pm while Child #1 remained on the playground. Child #1 ran to the front door of the center and began knocking on the glass approximately 30 seconds after the class entered the building. At approximately 3:30pm, a parent of another child at the center found Child #1 standing at the front door and rang the doorbell. Staff #2 answered the door at approximately 3:31pm and then escorted Child #1 to their classroom. Child #1 was out of sight and sound supervision for approximately two minutes.

Plan of Correction: Re-trained. Implemented name to face counts.

Standard #: 8VAC20-780-350-B-3

Evidence: On 10/25/2023 from approximately 3:29pm to 3:31pm there were 10 children present with Staff #1. However, the youngest child in the Threes group at that time was Child #1 (Age 2) making the ratio 1 staff to 8 children.

Plan of Correction: Had parents return the previously singed early promo.
