Comments:

An unannounced monitoring inspection was initiated and completed on 12/12/2023. The inspector was on site from 2:40pm until 3:25pm. There were 17 children, ages 5 years to 11 years old under the supervision of two staff. Children were observed during indoor free play and snack time. Three children's records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Description:	Based on record review, the center failed to obtain a copy of the results of a search of the central registry for any founded complaints of child abuse or neglect within 30 days of employment. Evidence: Staff #2, hired on 12/1/2022 had no record of a Central Registry on file during inspection on 4/6/2023.

Description:	Based on record review, the center failed to ensure all the following shall be kept for each staff person at the center.
