Technical Assistance:
Discussed allergy plans and asking parents to provide all medications listed to avoid a severe allergic reaction.

Discussed loose fitting crib sheets, cable attached to power lines that is buried in the playground.

Center should have parents include the strength of medication to be administered, not just the dose.

Two parents or guardians should be listed on enrollment paperwork. If a custody order is in place, the center should keep a copy in file.

Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 23 children directly supervised by 5 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, building with manipulatives, playing outside, and preparing for lunch. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on observation, areas and equipment inside of the center were not maintained in a safe and operable condition.
Evidence:
1. Drywall was observed to be peeling along the baseboards of the two year old room.
2. Baseboard heaters were observed in the infant room with gaping panels, exposing interior components.

Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
A spray bottle of bleach was observed in an unlocked cabinet in the bathroom used by children.
