A monitoring inspection was conducted today from 1:00 pm to 2:25 pm. There were 2 children in care of 2 staff members. The children were observed eating snack and engaged in free play. There were no children with allergies or medications on site. 3 staff records, 3 children's records, attendance, emergency drill log, emergency preparedness plan and other center documents were reviewed. Standard #: 22.1-289.035-A Description: Based on records review, the center did not obtain repeat background checks for each employee every five years.
Evidence:
1. Staff # 1's fingerprint based criminal record check (CRC) was last completed on 6/5/18. A repeat CRC was not obtained in five years.
2. Staff # 2's fingerprint based criminal record check (CRC) was last completed on 10/11/18. A repeat CRC was not obtained in five years.
3.Staff # 3's fingerprint based criminal record check (CRC) was last completed on 8/14/18. A repeat CRC was not obtained in five years.
4. Staff # 3's most recent central registry search (CRS) was last completed on 12/11/17. A repeat CRS was not obtained in five years.Plan of Correction: All background checks will be updated and a follow up will be conducted with the licensing inspector.
Standard #: 8VAC20-780-160-C Description: Based on records review, the center did not obtain subsequent tuberculosis (TB) screening for employees, every two years.
Evidence:
1. Staff # 1's most recent TB screening was completed on 10/27/20.
2. Staff # 2's most recent TB screening was completed on 5/2/18.
3. TB screenings must be repeated every two years.Plan of Correction: Updated TB screenings will be obtained.
