Comments:
An unannounced monitoring inspection was conducted on 11/29/2023 from 2:14 pm to 5:30 pm. There were 70 children in care at the start of the inspection, ages eight months to 5 years, in six groups under the care of 15 staff. The Licensing Inspector reviewed four children?s records, five staff records, three staff records (background checks only), and other program records. The children were observed on the playground, quiet activities, washing hands, and having lunch.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review, the center failed to obtain documentation of current immunizations prior to a child attending, unless an exemption is met.

EVIDENCE:
Child 1 began attending the center on 9/2/2022 and the child?s record has no documentation of immunizations. On 11/20/2023 a notarized religious exemption form was obtained.

Standard #: 8VAC20-780-140-A
Description: Based on record review, the center failed to obtain a physical examination within 30 days after a child began attending, unless an exemption is met.

EVIDENCE:
Child 1 began attending the center on 9/2/2022 and the child?s record doesn?t have a copy of a physical examination. On 11/20/2023 a notarized religious exemption form was obtained.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to obtain some required information in children?s records.

EVIDENCE:
1. The record for Child 3 is missing emergency contact information. Two emergency contacts are required.

2. The record for Child 4 has only one emergency contact, and two are required.

3. The registration forms for Children 3 and 4 have no information regarding whether the children have allergies or intolerances to medications or any other substances.

4. The registration form for Child 3 shows there is a chronic health problem, however, it is not specified, and accommodations aren?t shown.

5. Child 1 has a documented start date of 10/25/2022 and the three-part parent agreement has never been signed. The three parts are authorization for emergency medical care, agreement to pick up an ill child, and agreement to notify the center if the child has been exposed to a communicable disease.

Description: Based on document review and interview, the center failed to have a current annual health inspection.

EVIDENCE:
The most recent Report of Environmental Sanitation from the local health department was dated 10/27/2022.

Standard #: 8VAC20-780-350-Q
Description: Based on observation, interviews and record review, the center failed to have a written assessment by the program director and program leader prior to assigning a child to a different age group of such age group is more appropriate for the child?s developmental level and the staff to children ratio and group size is for the established age group.

EVIDENCE:
Nine children in the toddler group were observed in the care of two caregivers. Child 1 was in the group, and one of the caregivers gave their age as 14 months. This was confirmed by record review. The record for Child 1 did not have a written assessment of appropriateness to place the child in an older group. The lack of assessment was confirmed by staff 9.
