Technical Assistance:

Comments:
An unannounced monitoring inspection was conducted with the administrative staff from 10:45 am to 1:15pm. There were 113 children present with 19 staff providing supervision. The ages of the children ranged from 3 months to 5 years. The teachers were observed reading books to the children during circle time and preparing for lunch. Lunch consisted of chickpeas and pasta in a tomato sauce, green beans, salad and milk.
Medications for six children were reviewed as well as 4 allergy care plans and a sample size of 10 injury/accident reports. Five staff and five children's files were reviewed during this inspection as well.The most recent fire inspection was dated 2/13/23 and the health inspection was dated 2/7/23.Four buses were inspected today. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, it was determined that hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
The Licensing Inspector observed 10 containers of germicidal and disinfectant solution containers in the room identified as "Staff". The door to this room was unlocked.

Plan of Correction: The door to this room was locked during the inspection.

Standard #: 8VAC20-780-510-E
Description: Based on observation and interview with administrative staff, it was determined that medication authorization from the parent had not been renewed every 10 work days nor was their a physician authorization for the long term medication.
Evidence:
The parents written authorization was for an antihistamine medication for child #1 on 9/15/23. The parent had not renewed this authorization every 10 work days nor was their a long term authorization for this medication from a physician.

Plan of Correction: We will update the medication authorization.

Standard #: 8VAC20-780-510-I
Description: Based on observation and interview with administrative staff, it was determined that prescription medication was not maintained in the original, labeled container.
Evidence:
The Licensing Inspector observed an Epinephrine for child #1 not in its original pharmacy container and prescription label. The center reported that they did not have the original labeled container.

Plan of Correction: We will obtain the original prescription box or return the medication to the parent.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview, it was determined that medication was not kept locked.
Evidence:
The Licensing inspector observed unlocked medication ( Epinephrine, allergy medication) in the Discovery ( 2/3 year old) classroom for child #1. The medications were stored in an upper cabinet.

Plan of Correction: The medication was locked during the inspection.
